Problems With Lists

I had a shitlist entry that has disappeared but is still being active. By disappeared I mean the place that is help by a name in the lists is taken and the host mask is not there, but it still bans the original hostmask that was there. I cannot delete the entry like a normal entry from the lists. Is there a way to get rid of it? Maybe a manual way to do it?

Press Alt-R and click on the Users tab. Look down the list for the host. Anyone with user level 5 is S-listed.
