It's usually with alias2.mrc or alias4.mrc
I'm not getting any of the sound alerts when I'm voiced,opped, pmed, if someone is banned/unbanned.
The error messages come I believe when one of these sounds is supposed to be played.
The 421 errors start coming once I'm connected to a server. (EFNet atm)
I installed the 3515-A fix patch but that didn't seem to fix things.

Sounds more like corrupted settings than corrupted mrc files. Try installing the Invision+mIRC pack from ircmadeasy.com to a NEW folder and do NOT import settings. See if you have the same problems and let us know.
